Payment and Insurance

Payment options:

  • Check: Always accepted.
  • Credit Card:  Visa, MasterCard, Discover, or American Express.
  • Online Payment Link.
  • CareCredit:  Apply online (www.carecredit.com) or in our office.

Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan before or during your visit. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Please contact your insurance company to learn the details of your plan.

Most insurance companies should respond to the claim within one to six weeks. Any remaining cost is your responsibility. Statements accrue an interest of 1.75% each month they are overdue. Additionally, if we are unsuccessful at reaching you by email or text for online payment, a $5 billing fee will be added to your overdue account balance. After 90 days, all overdue statements will be sent to collections.

If we are unable to verify your insurance before or during your appointment, the total cost of service will be due. If insurance is provided after your appointment and is active for your appointment date, we can submit on your behalf.

If you miss your appointment, or cancel within 24 hours of your appointment, a $75 missed appointment fee will be billed to your account.

Dental Insurance Plans:

Please call our office at Stephen Vultaggio DDS Phone Number (978) 371-2792 to ask any questions about insurance coverage, or contact your insurance company to learn more about your plan. We participate with select BlueCross BlueShield and Delta Dental insurance plans; it is your responsibility to verify that our practice currently participates with your plan.

There are variations to insurance plans, therefore, it is not possible for us to know the specifics of your dental plan. We can also submit to out of network insurances for partial coverage. It is important you check your plan for out of network benefit coverage. Out of network plans we have submitted to on behalf of our patients include Guardian, United, Cigna, and Aetna.

For major procedures we can submit a pre-estimate to determine the estimated cost based on your benefits. Pre-estimates are not a guarantee of coverage and take time to process with your insurance company.

For major procedures we collect a deposit for the estimated patient portion. Additionally we require a signed treatment plan which shows the cost and procedure for your appointment.
