Our Practice Information
Our highly trained dental team works together to deliver precise, attentive care in a calm and supportive environment.
Cancellations
Please cancel or reschedule your appointment with a minimum of 24 hours notice. Appointments cancelled within this time frame may result in a $75 fee billed to your account.
Deposits
Deposits are determined based on your insurance, pre-estimates, our office fees, or our membership plan pricing. Deposits are required to begin major procedures or cosmetic treatments.
Insurance
Insurance and coordination is the responsibility of the patient. We are happy to assist and submit claims on your behalf. Pre-estimates can be submitted per the request of the patient.
Forms
Forms can now be filled out online. All forms are required to become a patient at our practice and all forms must be filled out before your first new patient appointment begins. Please arrive 15 minutes early to fill your forms out.
Location
We offer reserved, free on-site parking and easy accessibility for all patients. Our office is open Monday through Friday, with select Saturday appointments. Located conveniently at 115 Thoreau st.
Treatment
Our goal is to provide you with thorough dental care. All treatments recommended are to keep your smile at its healthiest. If X-rays are declined at multiple appointments, our office may dismiss you as a patient.
Running Late?
We understand that delays can happen, and we offer a 10-minute grace period for late arrivals. After this time, your appointment may need to be rescheduled and a cancellation fee may apply.
Rescheduling
Our office will confirm. You can opt out of communications at any time by texting STOP to our number. We only text to confirm appointments, handle scheduling, and to remind you of your appointments.
Billing
We mail out bills weekly and in accordance with how quickly we receive your insurance claims. Due dates are typically two weeks and late fees apply to balances overdue after 30 days.
